VERIFICATION CONSENT

I consent to the CIO Association of Canada contacting the Treasury Board of Canada Secretariat, and/or my employing organization, to confirm my employment level, scope of authority and leadership equivalency for the purpose of verifying my eligibility for the CDSO.D™ designation. I understand that this information will be used only to administer my application and my designation.

PRIVACY NOTICE

CIOCAN collects the personal information in this application to verify your eligibility, administer the CDSO.D™ assessment, award and maintain your designation, and administer your membership, renewal and continuing professional education records. Information may be shared with the University of Ottawa’s Professional Development Institute to confirm program completion, and with the Treasury Board of Canada Secretariat or your employing organization to confirm your role level and leadership equivalency, in each case only as needed for those purposes. CIOCAN handles your information in accordance with its privacy policy.

FEES

An active CIOCAN membership is a requirement for obtaining and maintaining the Chief Digital Services Officer Designation.
A full annual CIOCAN membership is required.
Payment is due upon completion of your application review.
The CDSO.D™ assessment fee is payable to CIOCAN. Fees may be paid by you or by a sponsoring organization.

I hereby certify that all statements in this application and in any material filed in support of it are true, correct and complete, and that all material information has been disclosed. I have read and agree to comply with the CDSO.D™ Code of Conduct and the applicable professional practice standards. I understand that the CIO Association of Canada may reject my application for the CDSO.D™ designation based on any information in this application found to be inaccurate or misleading. I acknowledge that the CIO Association of Canada has sole authority to award, renew, suspend or revoke the CDSO.D™ designation, and I agree to CIOCAN’s review, suspension and revocation processes. I pledge to conduct myself in a manner of integrity, honesty and respect for individuals in the CIO Association of Canada community, and I understand that if I am found to act contrary to those values, and in particular to the CDSO.D™ Code of Conduct, I may lose my designation.
